We follow the highest clinical & safety standards that help protect you long after you have clocked out of your shift.Benefits$60-75k/yrMonday-Friday 8am-5pm!Multiple Major Medical Plans to choose from (Medical, Dental & Vision)401KLife Insurance (Basic, Voluntary & AD&D)Paid Time Off (Vacation, Sick & Public Holidays)Short Term & Long Term DisabilityTraining & DevelopmentIncredible CultureResponsibilitiesAssists in development, review and revision of the plan of care.Provides nursing care in accordance with physicians plan of treatment, individual plan of care, as authorized by clients payer source.Observes, records and reports reaction to treatment and any changes in clients condition to appropriate personnel and/or physician.Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.Participates in education programs, quality assessment activities, nurses meetings and other staff meetings as required.Provides oversight for making patient and personnel assignments.Provides oversight for referrals.Responsible for initiating patient discharge planning upon patient admission to care. Accurately and timely complete Discharge Summary at the time of patient discharge from agency services.Performs HHA supervisory patient visits as required.Performs PRN visits, work rotating weekend shifts, and available on call as required.Participates in quality improvement program to identify and report problems, identify problem indicators, and perform chart audits and review.Works with DON to communicate patient status and problems daily.Travel tp patients homes and outreach locations to provide care under the direction of the DON.Maintains compliance with all Phoenix policies, procedures, and regulations, including all federal and state laws.Performs all other duties as requested by supervisor.RequirementsMissouri or compact RN LicenseHome Health experience preferredMinimum of one year successful experience as a RNMust have and maintain current valid driver's license and proof of current liability insuranceMust maintain current CPR certificateAbility to lift 50 poundsAbility to communicate clearly and make self understood effectively in face to face interactions; articulate with accuracy to communicate using phoneStrong organizational and self-management skillsWere taking the journey with you, creating a New Beginning!Choose Phoenix, Apply today!Our mission is to offer New Beginnings and meaningful opportunities to our caregivers and clinicians while providing home care services to our clients built on innovation, skill, and Christ-like values of compassion, honesty, and patience.by Jobble
