Overview: The RN Care Manager II - IPCM plays a crucial role within
the interdisciplinary team, assessing, planning, advocating, and
coordinating care from admission to discharge to ensure a safe
transition after hospitalization. This role includes evaluating
patients' health requirements, creating personalized care plans,
and coordinating services to enhance health outcomes. Effective
communication with patients, families, and healthcare professionals
is vital. Additionally, this position is key in advocating for
patients' needs, promoting health education, and ensuring seamless
transitions of care across various healthcare environments.
Clinical expertise, critical thinking, and compassion are essential
for fostering patient well-being and improving healthcare results.

Qualifications: Education: Bachelor of Science in Nursing, required
Licensure: Active RN License in the state of employment, required
Experience: Minimum of 1 year in an acute care hospital setting,
required Required Certifications: BLS (Basic Life Support) at the
time of hire, or within 90 days of employment Preferred
