a time. POSITION OVERVIEW Our Nurse Practitioners (NPs) and
Physician Assistants (PAs) are essential contributors to our
organization’s success, assisting our partners in improving the
quality of life for their residents. Key responsibilities include:
Evaluating patients’ medical and mental health requirements and
developing a care plan that proactively addresses their conditions
and overcomes barriers to care. Ensuring adherence to regulations
set by local, state, and federal agencies concerning the clinical
services provided. Prescribing medications and arranging lab tests,
diagnostic procedures, and consultations. Monitoring patients’
adherence to treatment plans and adjusting those plans as
necessary. Collaborating with RN case managers as part of a
cohesive care team. EXPERIENCE, CREDENTIALS, AND EDUCATION
Required: FNP, AGNP, AHACNP, or PA licensure. Degree from an
accredited APRN or PA program. A minimum of 3 years of experience
in clinical nursing or rehabilitation with geriatric populations.
Experience with Electronic Health Records. Proficiency in Microsoft
applications, including Word, Outlook, and Excel. SUPERVISORY
RESPONSIBILITIES May be expected to provide training and guidance

Education Master's degree or higher. Licenses & Certifications APRN
