$10K Sign-on Bonus Assists patients in navigating through clinics, hospitals, outpatient centers, insurance and payment systems, patient-support organizations, and other components of the health care system. Nurse Navigator will assist patients in the transition of care process from the hospital to outpatient, perform required processes, complete medication reconciliation, ensure PCP visits occur, and assist in reducing readmissions. Patient Navigator activities designed to achieve these outcomes include: Coordinating appointments with providers to ensure timely delivery of diagnostic and treatment services, maintaining communication with patients, health care providers to manage appropriate utilization of care, and hospital care team to ensure appropriate discharge and disposition of care
