Job Description:
This PRN Nurse Practitioner role involves conducting home risk
assessments for adult and geriatric patients, including reviewing
health, medical history, medication adherence, and social factors
during visits up to one hour.
Open to new graduates with at least 1 year of NP experience,
holding current licensure, board certification, and certifications
like BLS/ACLS.
Flexible scheduling includes days, evenings, nights, and weekends,
with travel required to patient homes, SNFs, and community settings
in Whitesburg, KY.
Compensation ranges from $1,800 to $9,000/month based on completed
assessments; mileage reimbursement provided.
Responsibilities include assessments, documentation, patient
education, referrals, and collaboration with healthcare teams.
